Why we ask for demographic information

When you seek care at Legacy Health, or visit us at a community event, we may ask questions about any disabilities or functional difficulties you experience and your spoken and written language. 

If there is a clinical need, we may also ask about your racial and ethnic identity and your sexual orientation and gender identity.

We ask for this information because we care that:

  • Your private medical record reflects your identity.
  • You receive individualized, high-quality care.
  • Our work promotes equitable care for the diverse populations we serve.

Answering the questions: voluntary and confidential

Answering the questions is voluntary. Choosing to answer all, some or none of the questions will not affect your access to services at Legacy. When you do answer, your information is considered Protected Health Information and becomes part of your confidential medical record.

What does Legacy do with this information?

We use data from all patients to identify care gaps and support health equity for all. We’re also required to report data to state health authorities for their own work on health equity. The information you share always stays confidential. Learn more about patient privacy

You can check any time in MyHealth or ask your care team next time you visit. You can also request a copy of your Legacy medical record.

How can I report an issue with personal information and my care?
